Application of blood transfusion sets in hemodialysis

I. As a Blood Transfusion Tool

1. Basic Function: A blood transfusion set is an infusion device that establishes a pathway between blood and a vein, safely and effectively delivering blood or blood products to the patient. During hemodialysis, if a patient requires a transfusion due to anemia or other reasons, the blood transfusion set becomes a crucial medical tool.

2. Operation Method: When using a blood transfusion set, first check that the packaging is intact, within its expiration date, and that the needle type is appropriate. After confirming everything is correct, follow the standard operating procedure for the transfusion, including venting air, venipuncture, administering a small amount of normal saline (to check vascular patency), and connecting the transfusion bag for the transfusion.

II. Assisting in Increasing Blood Flow

1. Application Background: During hemodialysis, sometimes patients experience insufficient blood flow, which may affect the dialysis effect. In such cases, a blood transfusion set can be used to assist in increasing blood flow.

2. Procedure: A transfusion set is connected to the dialysis tubing via direct arterial puncture (e.g., radial or dorsalis pedis artery) to increase blood flow during dialysis. This method requires strict adherence to procedures and specialized skills to ensure patient safety and dialysis effectiveness.

III. Applications in Special Circumstances

1. Heparin-free dialysis: Heparin-free dialysis is often used for patients with bleeding tendencies or active bleeding. In this case, a transfusion set connected to normal saline or other flushing solutions can be used to periodically flush the dialysis tubing to prevent clotting.

2. Handling venous sac clotting: During hemodialysis, venous sacs may become clogged with clotting. In this case, two transfusion sets connected to normal saline can be used to replace the clogged venous sac for blood return, reducing blood loss and restoring patency of the dialysis tubing.

IV. Precautions

When using a transfusion set for hemodialysis, strict adherence to aseptic techniques is essential to prevent cross-infection. Before a blood transfusion, a comprehensive assessment of the patient should be conducted, including the degree of anemia and bleeding risk, to determine suitability for transfusion and the appropriate amount and rate of transfusion. During the transfusion, the patient's vital signs and transfusion reaction should be closely monitored, and any adverse reactions should be managed promptly.
